WHAT IS YOUR NEW PATIENT PROCEDURE?

When you arrive, you will check in at the registration desk.  If a room is available, you will be placed in the room expeditiously.  One of our amazing nurses will obtain vitals and health history, and then shortly after that you will be seen by a provider.

I DON’T HAVE MY INSURANCE CARD WITH ME, CAN I STILL RECEIVE TREATMENT?

We will be happy to help you if you do not have your insurance card.  There may be associated costs with the visit, that could be refunded once you returned with your insurance card.

HOW MUCH WILL MY COPAYMENT BE?

Copayments vary depending on which insurance you have.  Often times this amount is located on the front of your insurance card.  If you are unable to locate it, please bring your insurance card with you, and we will happily assist in locating this for you.

WHAT IS YOUR NEW PATIENT PROCEDURE?

If you are a new patient, welcome!  As a new patient, there are a couple of extra forms that we will need you to fill out.  This just helps us get to know you better, and how we may best assist you.  Feel free to begin your registration process either online, or the paperwork may be provided to you at the time you check in.
